1.Describe your product and its intended use. 描述产品以及用途
List models, types or series. Outline any similarities & differences between products. 列出产品型号/序列号，概述每一种产品的相同之处于不同之处。

2. Identify each of the products components and materials by: 列出每种产品的构成与原材料
A - Manufacturer's name 工厂名称
B - Catalog number, size, rating 产品说明书号码、大小尺寸、级别
C - UL Listing or Recognition 是否有过UL的鉴定和推介，如果有，请列出来
D - Polymeric material used, with an explanation of usage and pertinent data (generic name, manufacturer & type designation) 所用的聚合材料的明细和他们的使用说明以及相关的数据（总的原料名称、供货的工厂名称、设计类型）

3. Include if available: 如果有可能的话，请提供更多的材料：
A - Schematic diagrams illustrating electrical circuitry 有图解说明的电路原理图
B - Photographs or sketches 照片或者素描
C - Sales brochures or catalogs 销售使用的说明书
D - Manuals, operation/installation instructions 使用指南、操作说明书、组装说明书等。

4. Describe, if available, any alternate construction(s) planned for future production. Include alternate parts, materials or arrangements of parts. 如有可能的话，请描述将来的替代产品，包括未来需要改进的某部分的零部件，原材料，以及零部件的新型组合

5. Include name, address, telephone, e-mail and fax number of the: 提交者的名字、地址、电话、传真、电子邮件箱等联络资料：
